{"id":304,"date":"2015-04-27T15:54:02","date_gmt":"2015-04-27T15:54:02","guid":{"rendered":"http:\/\/brooksidepress.org\/nursing_fundamentals_1\/?page_id=304"},"modified":"2023-08-12T20:22:25","modified_gmt":"2023-08-12T20:22:25","slug":"3-04-nursing-intervention-to-prevent-skin-breakdown","status":"publish","type":"page","link":"https:\/\/brooksidepress.org\/nursing_fundamentals_1\/?page_id=304","title":{"rendered":"3.04 Nursing Intervention to Prevent Skin Breakdown"},"content":{"rendered":"<p>The time of the patient&#8217;s bath or back massage is the most logical time to thoroughly observe the patient&#8217;s skin for pressure areas.<\/p>\n<p>At the first sign of redness, the area should be washed with soap and water and rubbed with lotion; measures should then be taken to keep the patient off the reddened area.<\/p>\n<p>Report any signs of pressure to the charge nurse.<\/p>\n<p>Keep sheets under the patient clean, smooth, and tight to help eliminate skin irritation.<\/p>\n<p>Ensure adequate nutrition and fluid intake, according to physician&#8217;s orders.<\/p>\n<p>Every effort should be made to keep urine and feces off the patient&#8217;s skin, washing the skin with soap and water and keeping the buttocks and genital area dry (lotion or powder may be used depending upon the patient&#8217;s skin type) when the patient is incontinent.<\/p>\n<p>Obese patients may need assistance washing and drying areas under skin folds (groin, buttocks, under breasts, and so forth.)<\/p>\n<p>For the patient with very dry skin, various bath oils may be added to the bath water.<\/p>\n<ul>\n<li>Soap may be omitted because of its drying effect.<\/li>\n<li>Lotions and oils may be used after the bath.<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>The time of the patient&#8217;s bath or back massage is the most logical time to thoroughly observe the patient&#8217;s skin for pressure areas.
