\/>\nalready invaded surrounding organs and
\n00:08
\ntissue in cases such as this the
\n00:11
\ndebulking surgery may be the best
\n00:12
\ntreatment option debulking surgeries are
\n00:15
\nperformed in an effort to remove as much
\n00:16
\nof the tumor as possible with the hope
\n00:19
\nof improving patient prognosis and
\n00:20
\nmaking them better candidates for
\n00:22
\nchemotherapy many patients with advanced
\n00:24
\novarian cancer often have severe ascites
\n00:27
\nas seen here in this case a large
\n00:32
\nmalignant tumor was noted to have
\n00:33
\ninvaded the omentum after blunt
\n00:43
\ndissection a vessel sealer was used to
\n00:46
\ncauterize a blood supply and then the
\n00:48
\ntumor and part of the omentum were
\n00:51
\nremoved and sent for pathologic
\n00:52
\nevaluation
\n01:02
\na Bookwalter retractor was then set up
\n01:09
\nto hold the abdominal walls apart the
\n01:21
\nuterus ovaries and any overlying tumors
\n01:23
\nwere then isolated with clamps sharp and
\n01:28
\nblunt dissection was used to separate
\n01:30
\nthe entire specimen from the surrounding
\n01:32
\ntissue and then the entire specimen is
\n01:35
\ncarefully removed from the abdominal
\n01:37
\npelvic cavity a large tumor is seen on
\n01:42
\nthe right ovary and the body of the
\n01:44
\nuterus is noted to be extremely firm to
\n01:46
\npalpation when the uterus is cut open
\n01:56
\nits body is noted to have a fleshy
\n01:59
\nconsistency which is suggestive of a
\n02:01
\nfibroid however the tumor on the right
\n02:07
\novary is highly suspicious for
\n02:09
\nmalignancy
\n02:20
\nanother tumor was visualized on the
\n02:22
\nsmall intestine after attempting to
\n02:25
\nmanually dissect it it was determined
\n02:27
\nthat a small bowel resection would be
\n02:29
\nnecessary in order to optimally debulk
\n02:32
\nthe tumor a GI a stapler was first used
\n02:36
\nto divide the affected portions of the
\n02:38
\nsmall intestine and once a tumor was
\n02:48
\nisolated from the small intestine
\n02:49
\nhemostasis was achieved using a large
\n02:52
\nvessel cautery device the tumor was then
\n02:55
\nremoved and sent to pathology
\n03:06
\nthe remaining end of the small intestine
\n03:08
\nare then carefully aligned and
\n03:10
\napproximated using multiple alice clamps
\n03:13
\na GI a stapler was then used to create a
\n03:16
\nside-to-side and nasty Moses
\n03:30
\nprior to closing the anastomosis is
\n03:33
\nthoroughly inspected to ensure proper
\n03:35
\nalignment<\/p>\n
<\/div>\n","protected":false},"excerpt":{"rendered":"
Duration
