1. Purpose. To establish Semper Fit, a comprehensive Marine Corps health
promotion program, as required by reference (a). This program encourages
healthy lifestyles which result in increased productivity, reduced medical
costs, and greater military readiness. Semper Fit consolidates programs
already mandated by references (b) through (h). These existing programs
should be used as resources in the establishment of local Semper Fit
Programs. The initiative behind Semper Fit is the Healthy People 2000
program, a program directed by the Department of Health and Human Services to
improve the health of our nation. Objectives from Healthy People 2000 have
been derived for the Department of Defense. These objectives have been
tailored as they pertain to the Marine Corps and are found in enclosure (2).

2. Cancellation. MCO 6200.4.

3. Background. The Marine Corps has a responsibility to its personnel,
military (active, reserve, and retired), civilian, and family members, to
maintain and encourage a high state of health and military readiness.
Approximately 50 percent of all deaths and illnesses in the United States
relate directly to unhealthy lifestyle habits: primarily tobacco use; poor
diet; lack of exercise; alcohol abuse; and unmanaged stress. Additional
compromises to health and productivity result from undiagnosed or
inadequately controlled hypertension (high blood pressure) and injuries.
Positive lifestyle and behavioral changes should result in optimal health,
an enhanced quality of life, and improved combat readiness for the Marine
Corps. Semper Fit is a combination of health education and related
organizational, social, economic, and health care interventions designed to
improve and protect health. Operationally, the health promotion program
includes nine elements: tobacco use prevention and cessation; physical
fitness and sports; injury prevention; nutrition education; stress
management; suicide awareness; alcohol and substance abuse prevention and
control; early identification and control of hypertension; and Sexually
Transmitted Disease (STD) and Human Immunodeficiency Virus (HIV) prevention.

4. Commandant's Intent. A change is sought in Marine Corps culture.
Surveys show that tobacco use and excessive alcohol consumption are
widespread in our Corps. These and other unhealthy lifestyle choices
undermine and weaken the most precious asset we have: our Marines. We
require a Marine Corps environment which will foster peak performance through
maximal wellness; an "ultimate health and fitness club" which serves to
maintain the "ultimate weapon", the U.S. Marine. Leaders at all levels are
tasked to shape our Marine Corps culture through education and personal
example and by implementing the letter and spirit of this MCO.

5. Applicability and Scope. This Order applies to all Marine Corps
personnel (active, reserve, and retired) and family members. Civilian
employees, both appropriated and nonappropriated fund employees, may
participate on a voluntary basis. However, this instruction does not
override provisions of existing civilian collective bargaining agreements
until their expiration. Prior to implementing this policy for civilian
employees, activities must, where applicable, discharge their labor relations
obligations.

6. Policy. The Marine Corps will ensure total force and family readiness
and maximize individual performance by adopting a variety of programs which
encourage healthier lifestyles and a quality of life at least equal to that
of the nation which we defend.

7. Elements. Marine Corps policy for each element of the Semper Fit Program
is as follows:

a. Tobacco Use and Cessation. Provide a safe, healthy, and unpolluted
environment which supports abstinence and discourages the use of tobacco
products per reference (b). The Marine Corps discourages the use of all
tobacco products in Marine Corps facilities, and employs a positive
educational and awareness approach designed to improve our state of health
and military readiness.

b. Physical Fitness. Provide opportunities and incentives to establish
healthy and active lifestyles in order to achieve and maintain optimal
physical fitness and body composition standards per references (c) and (i).

c. Injury Prevention. Provide education to reduce the incidence of
injuries in accordance with references (j) and (k). These programs will
assist in establishing and maintaining proper posture, body mechanics,
lifting skills, and safe work environments.

d. Nutrition Education. Provide comprehensive nutrition education
programs to achieve and maintain an optimal level of nutritional health and
body composition per references (c) and (e).

e. Stress Management. Provide programs to develop adaptive coping
skills to effectively deal with stress per reference (f).

f. Suicide Awareness. Provide annual training in suicide awareness.
Training should be provided to the smallest possible element (i.e., platoon,
section, etc.) to gain maximum effectiveness in this important element.

g. Alcohol and Substance Abuse Prevention and Control. Provide programs
and initiatives that prevent alcohol/substance abuse and/or addiction,
address early intervention, and provide substance and alcohol abuse
rehabilitation to break the cycle of addiction per references (d),(l) and
(m).

h. Hypertension Education and Control. Provide programs for early
hypertension identification, information on factors influencing blood
pressure (e.g., diet, exercise, medications) and treatment referral indicated
in reference (d).

i. STD and HIV Prevention. Provide annual training programs that focus
on modes of transmission and prevention of STDs and HIV per reference (h).

8. Organizational Objectives. Semper Fit will be administered by all
commands. Major commands listed in enclosure (3) will assign a Semper Fit
Coordinator (SFC) to accomplish the operation and implementation of the
Semper Fit Program. A guideline for the duties of a SFC is located at
enclosure (4). Subordinate units will assign Semper Fit Officers (SFO) so
that the program can be implemented and maintained at the lowest echelon.
SFOs are authorized to be noncommissioned officers for the purposes of this
program. In addition, Marines, vice Navy Medical personnel, are recommended
to be assigned as SFCs and SFOs.

a. Commands will utilize local medical, chaplain, Substance Abuse
Control Officer, and training personnel to the fullest extent possible at the
unit level for instruction and coordination of Semper Fit. External
resources from the Medical Treatment Facility (MTF), Substance Abuse
Counseling Center (SACC), Family Service Center (FSC) or other organizations
may be utilized as needed.

b. Specialized services such as tobacco cessation classes, injury
rehabilitation classes, etc., which are beyond the capability of locally
assigned medical or training personnel will be sought from available
resources such as the FSC, MTF, clinics, and hospitals through the SFC.

c. SFCs and SFOs may seek assistance from many other available resources
as well. A list of several suggested resources is provided at enclosure (5).
Liaison with CMC (MHH) and other major command SFCs is encouraged.

9. Program Requirements. The ultimate goal of Semper Fit is to create an
atmosphere within the Marine Corps that contributes to total health. To
achieve this atmosphere, all Marines must understand the importance of a
healthy lifestyle. Through proper education and training the goal of total
health will be easily attainable. The following guidelines are set to aid in
keeping health and fitness paramount in the Marine Corps:

a. Marine Corps commands must ensure that all Marines receive education
and training for 1 hour annually in Suicide Awareness, and STD/HIV Prevention
as well as annual training on Alcohol and Substance Abuse Prevention and
Control as required by reference (d). In addition, commands must ensure that
3 additional hours of Semper Fit training are provided annually. The 3
additional hours may be divided among the remaining Semper Fit elements.
Choice of training should be based upon the needs and interests of the
respective units. For example: a unit with a high incidence of tobacco
usage should devote part of their additional training to focus on the tobacco
cessation element. One of the hours may also be devoted to a general
orientation/introduction to health promotion as well.

b. All commands must ensure that the following mandatory training
programs/requirements are met for each of the following elements.

(1) STD and HIV Prevention. Ensure all Marines receive 1 hour of
standardized annual training on STD and HIV prevention. Lesson guides with
audiovisual materials available through major command SFCs should be
utilized. Training will be provided to all Marines, as well as paid work
time for civilian employees to attend the subject training once annually.

(2) Suicide Awareness. Ensure all Marines receive 1 hour of
standardized annual training on suicide awareness. Lesson guides with
audiovisual materials available through major command SFCs should be
utilized. Training will be provided to all Marines, as well as paid work
time for civilian employees to attend the subject training once annually.

(3) Alcohol and Substance Abuse Prevention and Control. Provide
training on alcohol and substance abuse prevention and control. Lesson
guides with audiovisual materials available through major command SFCs should
be utilized. Training will be provided for all Marines and civilian
employees in accordance with reference (d). Major commands will provide
direct support through advertising and/or pro bono use of facilities, support
groups which help alcoholics, addicts, and their families with recovery.

c. All commands should ensure the following additional programs/training
are available.

(1) Tobacco Use and Cessation. Provide training on tobacco awareness
and on smokeless tobacco awareness. Lesson guides with audiovisual materials
available through major command SFCs should be utilized.

(2) Physical Fitness. Provide training on physical fitness.
Standardized lesson plans and/or audiovisual programs to present the
information are available. In addition it must be ensured that facilities
are available and programs exist for all personnel to maintain physical
fitness as well as participate in command sponsored fitness and sports
programs/events. This will be accomplished through establishing formal
policy and publicity campaigns to encourage participation, by all personnel,
in fitness programs. Instruction and training on the benefits of regular
exercise should be available to all personnel, either through Marine Corps
personnel training for active duty Marines and civilian personnel or through
non-funded training for retirees, and family members. Weight control issues
and physical fitness requirements are set forth in references (n) and (i).

(3) Injury Prevention. Provide training on injury prevention.
Lesson guides with audiovisual materials available through major command SFCs
should be utilized as well as guidance provided on safety in references (j)
and (k). Training will be provided for all Marines and civilian employees
once annually.

(4) Stress Management. Provide training on stress management.
Lesson guides with audiovisual materials available through major command SFCs
should be utilized. Training will be provided for all Marines and civilian
employees once annually. Suggested training opportunities include pre/post
deployment and holiday periods.

(5) Nutrition Education. Provide training on nutrition education
and body weight management. Lesson guides with audiovisual materials
available through major command SFCs should be utilized. Individually
tailored weight control or dietary consultation should be sought as needed
from the local medical treatment facilities. Weight control support groups
will be provided, through advertising and/or pro bono use of facilities, to
applicable military personnel, families, retirees, and civilian employees.

(6) Hypertension Education and Control. Encourage all Marines to
receive biannual blood pressure checks and all other personnel to check their
blood pressure annually.

10. Reporting Requirements

a. All training completed in accordance with this Order should be
annotated in individual training records, for commands which use such a
system, to provide a personal record of training completion for every Marine.

b. Completion and recording of annual training requirements will be
inspected by Commanding Generals on regularly scheduled inspections.

11. Action

a. As program sponsor, CMC (MH) will:

(1) Coordinate the activities of Health Services (HS), Personnel
Management Division (MM), Training and Education Division (MCCDC), Safety
Division (SD), Manpower Plans and Policy Division (MP), Chaplain of the
Marine Corps (REL), Public Affairs (PA), Morale, Welfare, and Recreation
Support Activity (MW), Facilities and Services Division (LFS), and research
in support of the Semper Fit Program at the Headquarters, SECNAV, and DoD
levels.

(2) Provide guidance to all activities regarding Semper Fit policy
and plans.

(3) Provide information on publicity/marketing programs, plans, and
materials.

(4) Develop and recommend program evaluation and effectiveness plans
to all activities.

(5) Provide standardized lesson plans with audiovisual materials to
all major command SFCs.

(6) Assess progress toward goals and objectives, set forth in
enclosure (2), in conjunction with DoD and National Health Objectives.

(7) Create an awareness designed to publicize the benefits from
Semper Fit and target efforts to celebrate Semper Fit, in health-oriented
publications via internal and external release efforts.

b. CGs MCRDs will:

(1) Prohibit the use of tobacco products by staff and instructors
while in the presence of recruits at all initial entry and training points.

(2) Provide instruction on the Semper Fit Program to all recruits.
Providing this instruction in addition to existing training is not necessary.
However, incorporating an overview of Semper Fit and referring to it, as it
applies to current training (ie; HIV and suicide awareness), is required.

(3) Ensure that suicide awareness, and HIV/STD prevention is
included in the training for all recruits.

c. CG MCCDC will:

(1) Ensure elements from Semper Fit are included in the Troop
Information Program as per reference (g).

(2) Provide Semper Fit training, suicide awareness, and HIV/STD
prevention, to all officer candidates and all officers under instruction as
per references (h) and (o).

(3) Prohibit the use of tobacco products by staff and instructors
while in the presence of students at all initial entry and training points.

d. CMC (LFS) will: Ensure that food service personnel develop menus
that promote healthy food choices and support health enhancing decisions

in the food service environment. Reference (e) applies.

e. CMC (MW) will:

(1) Advise MWR activities to incorporate the use of commercial and
community sources, as per reference (p), in support of their local Semper
Fit program as appropriate.

(2) Promote healthy choices in food and hospitality facilities to
facilitate the well-being and morale of Marines, their families, and
authorized patrons as per reference (p).

(3) Encourage local MWR activities to promote and support the Semper
Fit Program as it pertains to the conduct of their missions.

f. CMC (REL) will:

(1) Provide advice and assistance regarding Chaplain and Command
Religious Program involvement in support of Semper Fit.

(2) Ensure that Command Religious Program personnel, in cooperation
with the local MTF, FSC, and SACC are a primary resource to assist local
commands in developing stress management and suicide awareness.

g. CMC (MP) will:

(1) Ensure that civilian employees are encouraged to pursue healthy
life styles.

(2) Respond to civilian employee inquiries and provide advice and
guidance via the normal chain of command with respect to civilian personnel
policies and issues related to the Marine Corps Semper Fit.

h. CMC (PA) will:

(1) Disseminate, through all available means, to internal and
external audiences, the sincere concern for Marines and their families as
reflected by the Semper Fit Program.

(2) Ensure that Marine Corps internal media sources incorporate
periodic Semper Fit information as part of their overall production planning.

i. CMC (HS) will:

(1) Advise and assist the involvement of FMF medical departments in
providing support to the Semper Fit Program.

(2) Request from the Chief of Bureau of Medicine and Surgery the
assignment of a Semper Fit Medical Liaison at each MTF located on Marine
Corps installations. This individual will be given the collateral duty as
Semper Fit Liaison and assigned to the CG of the respective installations.

j. Commanders of those commands listed in enclosure (3) will:

(1) Assign, in writing, a SFC.

(2) Assign health promotion responsibilities, as outlined in
enclosure (4), to command SFCs and provide training to assist them in
their duties.

k. Commanders of Marine Corps installations and will:

(1) Form a command committee to oversee and facilitate the Semper
Fit Program. This committee should include, at a minimum, representatives
from: the FSC, SACC, MWR, MTF, safety office, and the Chaplain.

(2) Assign a Semper Fit Medical Liaison, from the local MTF, to
provide support to SFCs with medical resources necessary to the program.

(3) Assign liaison at the FSC to provide support to SFCs with FSC
resources necessary to the program.

(4) Assign a liaison at MWR departments to make liaison with local
command SFCs in order to implement the Semper Fit Program and increase the
awareness of the objectives contained in enclosure (2).

(5) Coordinate the use of Non-Appropriated Funds, from local MWR
resources (reference (p)), for use by major commands to successfully
implement and operate the Semper Fit program.

l. Commanding Officers at the battalion/squadron level and above will:

(1) Assign, in writing, a SFO. The SFO should be a Staff
Noncommissioned Officer or above.

(2) Ensure compliance with program direction from the SFC and local
Semper Fit committee.

(3) Assure required Semper Fit training is provided to all
personnel.

m. All commanders will:

(1) Use assigned medical assets to the fullest extent possible in
carrying out Semper Fit training at the unit level.

(2) Include Semper Fit Program compliance and evaluation as part of
the command inspection program.

(3) Sponsor and/or participate in local community activities that
enhance and encourage physical fitness (especially youth oriented activities)
in support of Semper Fit Programs.

(4) Ensure command orientation programs include information on where
to obtain Semper Fit assistance and resources.

(5) Promote a smoke-free working environment in accordance with
reference (b), and issue a written tobacco use policy which includes a list
of designated smoking/smokeless tobacco use areas.

(6) Encourage sponsorship or participation in an annual health fair
and any safety awareness promotion. Health fairs are encouraged to coincide
with National monthly health themes such as the National Hypertension and
Physical Fitness and Sports Month (May). Assistance is available from local
MTFs, FSCs, and MWR activities.

(7) Use command religious programs, FSCs, SACCs, and local MTFs as
the primary resources for development of command stress management programs.
Address stress management by ensuring:

(a) The availability of programs designed to support personnel
and their families as they deal with day to day stresses per reference (f).

(b) That major subordinate commands use Critical Incident Stress
Debriefing (CISD) teams if available. CISD personnel work in conjunction
with the Chaplain and command personnel on the unit level in peacetime as
well as combat to focus on normalizing reactions of personnel and educating
them about the psychological and emotional responses they might experience
following incidents which affect some or all unit personnel (i.e., suicides,
vehicular accidents resulting in death, aircraft mishaps, etc.). Reference
(f) applies.

(8) Promote leadership practices that reduce job related stress
through programs that enhance a condition of

mental alertness and physical well-being among members of their command
(e.g., quality of life policies, equal opportunity, sexual harassment, MWR
programs, counseling services, etc.).

(9) Ensure the existence of a work environment conducive to
improving and protecting health.

(10) Provide an environment that promotes the message: "It's OK to
get help." Personnel experiencing difficulties in a break-up of a
relationship, alcohol abuse, managing stress, etc., should be referred to an
appropriate resource for counseling and supportive services without prejudice
to their career.

n. All personnel are ultimately responsible for their own lifestyle
choices and physical readiness. All personnel will become familiar with the
Semper Fit policies, objectives, and assistance resources and will comply
with the conditions set forth in this Order.

SEMPER FIT
GOALS AND OBJECTIVES

1. The Marine Corps Semper Fit Program objectives are based on health
objectives adopted from the Department of Health and Human Services Healthy
People 2000 Program. The areas addressed are: tobacco use prevention and
cessation, physical fitness and sports, injury prevention, nutrition
education, stress management, suicide awareness, alcohol and substance abuse
prevention, hypertension screening/education and control, and STD/HIV
prevention.

2. The main source of information to determine progress toward goals and
objectives will be the Department of Defense (DoD) Survey of Health Related
Behaviors Among Military Personnel. This survey is conducted approximately
every 3 years and is designed to establish baseline data to assess progress
toward selected Healthy People 2000 objectives for active duty military
personnel. Any reference to the survey in this enclosure will refer to the
DoD Survey of Health Related Behaviors Among Military Personnel only.
Participation in this survey is completely voluntary and the names of
participants are held confidential.

3. The focus of health promotion in the Marine Corps will be on continuous
improvement above and beyond those goals set in the Healthy People 2000
program. However, the objectives listed below are those determined for the
Marine Corps to achieve by the year 2000. Updated and improved goals and
objectives will be promulgated as future DoD Surveys determine progress.

4. Goals and Objectives

a. Tobacco Use Prevention and Cessation

(1) Goal. Reduce tobacco use.

(2) Objectives

(a) Increase to 80 percent the number of Marines who report on
the survey that they are non-smokers.

(b) Increase to 90 percent the number of Marines who report on
the survey that they do not use smokeless tobacco.

b. Physical Fitness

(1) Goal. Increase physical fitness.

(2) Objective. Implement a Physical Conditioning Program (PCP) in
accordance with reference (i), that requires a minimum of 3 hours of physical
activities per week that promotes the development and maintenance of
cardiorespiratory/muscular fitness.

c. Injury Prevention

(1) Goal. Reduce injuries.

(2) Objective. Reduce work related injury resulting in
hospitalization to less than 3 cases per 100 personnel.

d. Nutrition Education

(1) Goal. Reduce cholesterol levels.

(2) Objective. Reduce, to 6 percent, the number of Marines who
report on the survey that they have been told by a health professional that
they had high cholesterol.

e. Stress Management

(1) Goal. Reduce productivity loss due to stress.

(2) Objective. Increase, to more than 75 percent, the number of
Marines who report on the survey that they experienced no decrease in job
performance due to stress at work or in the family.

f. Suicide Awareness

(1) Goal. Decrease suicides.

(2) Objective. Reduce the average Marine Corps suicide rate to less
than 15 per 100,000 personnel.

g. Alcohol and Substance Abuse Awareness

(1) Goal. Reduce alcohol abuse.

(2) Objectives

(a) Reduce to 15 percent the number of Marines who report on the
survey that they are heavy drinkers (defined as consuming 5 or more drinks
per occasion each week).

(b) Decrease to 12 percent the number of Marines who report on
the survey that their job performance was affected due to alcohol consumption
(i.e., leaving work or late for work due to a hangover, lack of concentration
due to intoxication, etc.).

h. Hypertension

(1) Goal. Decrease hypertension.

(2) Objectives

(a) Increase to 100 percent the number of Marines who report on
the survey the results of their blood pressure checks within the past two
years.

(b) Reduce to 8 percent the number of Marines who report that
they have been told by a health professional that their blood pressure was
high.

i. STD and HIV Prevention

(1) Goal. Reduce the occurrence of STDs and HIV.

(2) Objectives

(a) Increase to 75 percent the number of sexually active
unmarried Marines who report on the survey the use of condoms.

(b) Reduce the average incidence of new Marine HIV cases to less
than 31 cases annually.

(c) Decrease, to 18 percent, the number of Marines who report on
the survey that they have had a sexually transmitted disease in their
lifetime.

DUTIES OF THE
SEMPER FIT COORDINATOR

1. The command SFC shall be assigned in writing. In addition, the billet
does not require an officer to fill the duties. Duties can be determined by
each command, however, a basic guideline of Coordinator duties are as
follows:

a. Advise the CG or CO on all matters pertaining to health promotion and
wellness.

b. Prepare and provide standardized educational material for unit level
training in each of the areas specified in the Order.

c. Form a command committee of key personnel whose agencies/areas are a
part of Semper Fit responsibility, (i.e., SACC, MWR, FSC, MTF, etc.)

d. Assist in providing administration and logistics support for Semper
Fit training/events.

e. Educate personnel on the elements of Semper Fit.

f. Maintain contact with members of the local Semper Fit network to
include: MTF, SACC, FSC, Chaplains, safety office, lower echelon
coordinators, and MWR offices.

g. Ensure compliance and assistance from all members of the Semper Fit
network.

h. Keep the Commanding Officer abreast of all training requirements and
completions.

i. Oversee the development of command programs for involvement of
Marines, family members, and civilian personnel.

j. Provide training to those coordinators assigned at lower echelons on
the aspects and functioning of the Semper Fit Program.

k. Identify immediate, intermediate, and long-range needs of the command
in order to reach goals set by this Order in enclosure (2).

2. Provide reports to CMC (MHH), as required by this Order.
